Meet the Staff
Matthew Horne, PT, CSCS is a 1998 graduate of the University of Vermont. He received his Strength and Conditioning Specialist Certificate in 2002, and he & Tracy are the owners of Addison Wellness, Inc. Matt’s professional experiences have included inpatient acute care, home health and advanced work in outpatient orthopedic and sports rehabilitation. Matt currently lives in Middlebury with his wife and two daughters.

Tracy Horne, PT is a 1998 graduate from University of Vermont. She has worked in acute care, home health and long/short term rehabilitation with the majority of her experience in outpatient rehab. She has extensive training in postural repositioning, maximizing biomechanical function, alignment, recovery and strength. She lives in Middlebury with her husband and two daughters. She enjoys soccer, skiing, gardening, reading and spending time with family and friends.
Kristi Myers, PTA graduated from Idaho State University in 2012 with a Bachelors degree in Health Science and an Associates as a Physical Therapy Assistant. Her clinical experience has been in outpatient Physical Therapy. She is a certified Personal Trainer & Group Exercise Instructor and loves almost everything fitness related. She enjoys weightlifting, teaching group classes and spending time with her husband David.
Lauren Peterson, Clinic Assistant, Receptionist & Marketing Assistant joined the Wells Physical Therapy team in December 2013. Originally from San Diego California, Lauren is enjoying the Vermont Lifestyle. She has grown to love and appreciate all that a small town community has to offer, especially that it gives her the opportunity to establish friendly and caring relationships with our patients. When she’s not at work, Lauren enjoys her time with her daughter and two crazy pups exploring the VT countryside.
Lisa Perkins, Billing Specialist
An interest in natural therapies led Lisa to administrative and billing work in the fields of chiropractic, naturopathy and eventually physical therapy. Lisa joined the Wells PT team in 2009. She lives with her husband and two daughters in Monkton, and in her spare time enjoys reading, biking, camping, gardening and ghost hunting.
