Meet The Team
Matthew Horne, PT, CSCS is a 1998 graduate of the University of Vermont. He received his Strength and Conditioning Specialist Certificate in 2002, and he & Tracy are the owners of Addison Wellness, Inc. Matt’s professional experiences have included inpatient acute care, home health and advanced work in outpatient orthopedic and sports rehabilitation. Matt currently lives in Middlebury with his wife and two daughters.

Tracy Horne, PT is a 1998 graduate from University of Vermont. She has worked in acute care, home health and long/short term rehabilitation with the majority of her experience in outpatient rehab. She has extensive training in postural repositioning, maximizing biomechanical function, alignment, recovery and strength. She lives in Middlebury with her husband and two daughters. She enjoys soccer, skiing, gardening, reading and spending time with family and friends.
Jodi Eddy, PT is a 1998 graduate from University of Vermont. She returned to Vermont after 20 years of outpatient practice in southern New Hampshire and joined the Addison Wellness, Inc team in 2021. Jodi has been trained in Vestibular Rehabilitation and is certified in Functional Dry Needling and Instrument Assisted Soft Tissue Massage. Jodi is an avid fan of all sports and an outdoor enthusiast. She is grateful to be back in Vermont with her son and daughter.
Lisa Perkins, Billing Specialist
An interest in natural therapies led Lisa to administrative and billing work in the fields of chiropractic, naturopathy and eventually physical therapy. Lisa joined the Wells PT team in 2009. She lives with her husband and two daughters in Monkton, and in her spare time enjoys reading, biking, camping, gardening and ghost hunting.
Theresa LaCava, Part-time Receptionist
Terry joined the Addison Wellness team in 2021. She is retired from her job as a travel advisor but works to make some extra cash to buy dog treats for her little fur baby. At the clinic she enjoys meeting people and spending time in the Middlebury Community
